Disputed Records enquiry

If you believe that the information provided in your police certificate (Criminal History Check) is incorrect or inaccurate in any detail, you can apply to the AFP for a correction to the record.

You will need to provide the following details of the offences or information in dispute:

  • Arrest name
  • court
  • court date
  • outcome
  • supporting documents, if available
  • fingerprints (on request).

Dispute process

  1. Complete the Disputed Records form CR 1200 (Word doc) online and print it
    OR print the Disputed Records form CR 1200 (PDF, ** Kb), and complete it
  2. Sign and date the form
  3. Attach any supporting documents
  4. Send to the address shown on the form.

The AFP may require additional information or comparison fingerprints to resolve some disputed. You will be contacted if this is the case.
